Health Questionnaire when Embarking

I have noticed that not all Ports make you fill out the Health Questionnaire when embarking - Ft Lauderdale (Port Everglades and Vancouver to name two) while LA (San Pedro) is a stickler for it and is my first stop before I go up the escalator.

I thought it was a requirement and wonder if anyone knows why some ports don't require it?

We have sailed out of Ft Lauderdale several times in the past couple of years. We always had to fill out the health form. I think that it is required of all ports to issue one to each passenger. We also sailed out of Seattle and had to fill one out then and also in Bayonne, NJ.

I was issued this questionnaire once in Miami 2010 and no one was around to give it back to and it found its way in a trash can minus my name.

Carnival does provide them online which is great.. But they also update them so many times... Since I started working at the port in bmore, there have been 3 different forms... The form is now down to 2 questions.

I've only seen paper forms. My touchpad idea is pretty much a pipe dream. ~LOL~

It is a form as part of the online check-in for our upcoming Carnival cruise. I don't know about others.


The only cruise port in Fort Lauderdale is Port Everglades.

I have only sailed out of Miami and Fort Lauderdale (Port Everglades) and I had to complete at both.

I have filled one out for each cruise here in the USA and in Europe.

Maybe they’ve just all blurred together, but I seem to recall filling them out before every big-ship cruise for many years now – whether embarking in US, Canadian or European ports.

Its unfortunate that this could not be part of the online check in but timing needs to be immediate to the departure and people can check-in well in advance of their departure date.

If port agents asked the questions out loud to each passenger, it is a bit intrusive on individual privacy. Even though HIPAA does not apply, I would be uncomfortable overhearing other passenger's health-related responses or stating our own out loud.

Having touch pads at the check-in counter where each passenger would have to enter responses to each of the questions might be an option... but the expense of it is probably why cruise lines have not already instituted it.
