Health Questionnaire when Embarking

I have noticed that not all Ports make you fill out the Health Questionnaire when embarking - Ft Lauderdale (Port Everglades and Vancouver to name two) while LA (San Pedro) is a stickler for it and is my first stop before I go up the escalator.

I thought it was a requirement and wonder if anyone knows why some ports don't require it?

It is a form as part of the online check-in for our upcoming Carnival cruise. I don't know about others.

I've only seen paper forms. My touchpad idea is pretty much a pipe dream. ~LOL~

Carnival does provide them online which is great.. But they also update them so many times... Since I started working at the port in bmore, there have been 3 different forms... The form is now down to 2 questions.

I was issued this questionnaire once in Miami 2010 and no one was around to give it back to and it found its way in a trash can minus my name.

We have sailed out of Ft Lauderdale several times in the past couple of years. We always had to fill out the health form. I think that it is required of all ports to issue one to each passenger. We also sailed out of Seattle and had to fill one out then and also in Bayonne, NJ.
