If port agents asked the questions out loud to each passenger, it is a bit intrusive on individual privacy. Even though HIPAA does not apply, I would be uncomfortable overhearing other passenger's health-related responses or stating our own out loud.
Having touch pads at the check-in counter where each passenger would have to enter responses to each of the questions might be an option... but the expense of it is probably why cruise lines have not already instituted it.
