Where do you get the Bermuda authorization form at

I was there in April. They ask that you wear your mask while indoors, restaurants, shops, transportations. Don't forget to fill out your Bermuda travel authorization form and upload your results 2 days prior to getting on the boat.

You need to go on Bermuda travel and fill out the authorization form.I would go on NCL website
