You need to go where you got your vaccine and ask them to issue you a new one because you lost your original. Most medical facilities and pharmacies have it in their record.

If you have it on a verified health app like MyChart it will be accepted. Also accepted are pharmacy receipts as long as they show the vaccine batch number and administering professional's signature with your name and date received.

Go to where you got your vaccines or to your local county health department. They should be able to look it up and get you a new card.
