If for any reason you must fly home from one of the ports, you will need a valid passport. Reasons could be: miss the boat, get sick, accident, etc.

not if you're doing a closed loop cruise. starting and ending in US.

No, if you are on a closed loop cruise port. (starts in Galveston, TX and ends in Galveston, TX)
