Review summary
I really want to give this cruise a 5 star rating, but there were a few things that could have been better. First off we had a Handicapped room with a Balcony View and that was amazing, but the bathroom was not too clean. I took a shower and the hand held shower attachment that you can use when you sit down was not attached and when I switched over to using it, the water pressure pushed the attachment off and it hit me in the head. In the upper corner there was some type of bug nest. The grab handles around the bathroom were not cleaned. Only the middle of the handles had no dust build up on them. The little draw in the bathroom had not been cleaned out in quite a while so I would not use it. We had some hot nights in our room because our Air Conditioner just didn't seem to be working. The one mattress that had a pad with a cover, the cover was not on the pad and it was extremely lumpy. We had a couple of issues in the American Icon Restaurant with a Breakfast that we went to on 5th day of the cruise. We were seated for our Breakfast and we got our drinks. We ended up sitting there for 45 minutes and no one came over to offer us refills or even take our Breakfast order. We had to get up and leave because we wanted to go onto the Island. On the last night at sea, the crew was using the Incinerator and there really wasn't a breeze so the smoke just hung in the air around the boat. Well, it was a little nerve racking not knowing that that was what was going on. It would be nice if they would put a notification somewhere informing people of this. We were a little concerned when we opened our Patio door and saw and smelled smoke.

Buy yourself an over the door plastic shoe holder at Walmart for 12 dollars. We used it to hold our Bathroom items and other small things. it kept the clutter out of the room and the small counter in the Bathroom clutter free.

Great advice from jilesgulf. In addition to politely addressing service issues real-time, do complete the post cruise survey. We write names of our assigned service staff and those that stood out for good service.
With the slow service you mentioned, as with land based restaurants, we try to determine whether it is from inattentive staff or because the ship/cruise line understaffed the function either by design or due to an unplanned issue with the crew; I would rather have slow service than have an ill crew member provide service. When you left, a polite comment to the maitre'd might have resolved that for a repeat visit, maybe with a compensation of some kind.
The Ship's crew want your good reviews. Informing them of poor service and then giving them the opportunity to remediate and finally feedback during the cruise can be very effective.
