We really toned down our “door decorations” for our Princess cruise. We only put a magnet of the Colorado flag, just below the peep hole, on our door. Several fellow guests told us that it helped them know that they were close to their cabin. We were located right at a turn in the corridor. The door decorations on the doors on Carnival are one of the things that bring me smiles. I sometimes go down corridors on different decks, just to see what’s there.
Not sure that’s what DCDB meant by the original question, but personalizing doors is different than wearing a “t-shirt” with “Smith family reunion” or “Pinkie’s ?? birthday” on it.

We have a custom door magnet a friend bought for us years ago.
My wife usually makes us "______ of the Seas Bar Crawl" shirts that list all of the bars on the ship, but other than that...Not Really.
I had door magnets in the shape of flip-flops made for our scheduled 2020 cruise on the Carnival Magic. I did it to commemorate my daughter's 18th. birthday, and was intended as a memento for her and the friend we had her invite along. It detailed their names, the month/year and the ship. Unfortunately the month/year was March 2020, and we all remember what happened at that time. Not to worry though. After many delays and subsequent cancellations, we took the same cruise in March of this year. So being ever resourceful, I purchased some number stickers and put a "2" over the second zero in 2020. Voila. Although it looked cheesy, everyone thought it was hilarious and my daughter & her friend, who are now roommates, have them on their refrigerator.
BTW: If anyone's interested, I got them from Esty. They looked similar to this:
We ordered custom bobble heads on one cruise.
We have a few "generic" custom items like: T-shirts with sayings "Caution I bough the drink package", "drinks well with others" & "boat hair don't care". Plus we have personallized an eracer board for our door. If that's what your thinking.
Not sure what you mean by custom items. Are you talking about things to remember the cruise like mementos? We have a few including a fridge magnet showing the itinerary and map for a cruise which was made by someone on the cruise.
