English is the primary language, especially signage. Much of the crew is multilingual. We did have one show though which seemed to be largely in Italian. They tried to throw in a few English, Spanish, and German words to keep others informed of the basic story, but was spotty at best. We are only English speakers and got by just fine. In much of the ports off-ship though, you may have communication difficulties.
