no for all. vaxxed or unvaxxed. Just like 2019 again.

Sometimes. Depends on ports of call, as you might be able to board the ship unvaccinated but can't visit certain islands and have to stay on board. Ports change requirements as local variants and hospitalizations fluctuate.
