Depends entirely on the service. Usually, yes and at the end of the cruise.
Throughout the cruise. And at the end. Same thing with bartenders I like. I get great service that way plus it makes me feel good.

Only once have we not tipped extra since the room steward did nothing above the minimum and was actually rude to DD. She was 26, but looked 16, and he was scolding her for putting the bunk up without his assistance. DH actually did that, and shouldn't have, but we didn't know that. He never said a word to DH or myself.
I have had good cabin stewards and not so good stewards. If they go above and beyond, I tip. Generally what I mean is if they are always courteous, remember my name and clean my room, which has sometimes not happened, then yes definitely.
We've always had pretty good stewards so I usually tip upon arrival and throughout the cruise if I have special request.
